According to the Alzheimer's Association's 2010 Alzheimer's Disease Facts and Figures, Hispanics are about one and one-half times more likely than whites to have Alzheimer's and other dementias. When Blue Cross Blue Shield of Michigan measured the rate of hospital admissions for patients with diabetes, chronic heart conditions, and asthma, the Detroit-based health plan found that hospitalizations for patients being cared for in a patient-centered medical home (PCMH) were 23% lower than for patients treated in other practices.
